[摘要] 目的 探讨人工桡骨小头假体置换手术治疗桡骨小头粉碎性骨折的适应证、临床意义及其早期临床效果。方法 自2007年-2010年，我们对11例肘部不同创伤引起的桡骨小头骨折予以人工桡骨小头置换术。其中单纯桡骨小头粉碎性骨折3例，行单纯桡骨小头置换；孟氏骨折5例，在尺骨内固定后行桡骨小头置换；肘关节“恐怖三联征”3例，尺骨鹰嘴、冠状突固定后行桡骨小头置换。按Schatzker和Tile分型皆属于C型骨折[1]。术后指导患者行正规肘关节康复功能锻炼。结果 所有患者皆获得了随访，随访时间3-18个月，平均12个月。按Broberg标准对患肘关节功能进行评价[2]：优8例，良2例，差1例，优良率91%。结论 对于Schatzker和TileC型骨折，使用人工桡骨小头假体置换可获得非常满意的近期临床效果，其远期疗效需进一步探讨和随访，是可供临床选择的最佳方法之一。宁夏医科大学总医院创伤骨科安维军

The clinical effects analysis and research of radial head prosthesis replacement surgery

[Abstract] Objective To investigate the indications、significance and early clinical results of radial head prosthesis replacement surgery for treatment of radial head comminuted fractures .Methods From 2007 -2 010 years, we have treated surgically 11 cases of radial head comminuted fractures that caused by different types of elbow trauma, the artificial radial head prosthesis replacement to be adopted. In all of the cases,the isolated radial head fractures 3 cases that performed prosthesis replacement only; 5 cases of Monteggia fractures that prosthesis replacement were done after ulna fractures were fixed; elbow "terrible triad" in 3 cases that prosthesis replacement were done after ulna and coronoid process fractures were fixed. According to Schatzker and Tile classification ： all belongs to C type fracture [1]。 Formal rehabilitation training of the affected elbow was guided after operation .Results  All of the cases were  followed up ,the time was 3-18 months, The average time was 12 months. the postoperating effect and the function of the affected elbow was evaluated  according to Broberg and Morrey criterion [2]：excellent in 8 cases, good in 2 cases and poor in 1，the excellent and good rate are 91%. Conclusions With regard to the Schatzker and Tile C fractures, using  artificial radial head prosthetic replacement to surgical treat the comminuted radial head fractures can acquire  very satisfied recent-term clinical results, the long-term effects need further study and follow-up , this is one of the best methods available for clinical choice.
