Local chemotherapy combined with percutaneous pedicle screw in adjacent vertebrae in the treatment of spine tuberculosis

【Abstract】: Objective To assess the clinical efficiency of local chemotherapy combined with percutaneous pedicle screw in adjacent vertebra in the treatment of spine tuberculosis. Methods From April 2007 to June 2011, 15 advanced spinal tuberculosis patients were treated by local chemotherapy combined with percutaneous pedicle screw in adjacent vertebrae. Among them, there were 6 males and 9 females. Their ages ranged from 36 to 85 years, with an average of 64 years. There were 8 in thoracic,4 in thoracolumbar and 3 in lumbar. The level of the lesion were 12 for two vertebrae and 3 for three vertebrae. 3 cases were complicated with paraplegia with 2 ASIA C and 1 ASIA D. Result All cases were followed up for an average of 13 months(range 3-53 months). 10 patients were cured and 4 patients were improved besides 1 case aggraved who were treated by anterior debridement with autograft bone fusion and spinal reconstruction. No pedicle screw and rods losse or fracture and no kyphotic angle increase occurred to the end of follow-up. Conclusion Local chemotherapy combined with percutaneous pedicle screw in adjacent vertebrae is one of method to treat spine tuberculosis,which has some advantages including fewer traumas,less blood lose, earlier action and no kyphotic angle increase..
